Child's Details

Please complete this form for each Nippers participant (age 13 or younger on 30th September). 

You do not need this form for:

  • Youth 14 and up. Find out about opportunities as junior coaches and leaders.

  • The all-abilities Starfish Nippers program. Sign-up is handled by the Starfish manager.

In which group will this child participate?
Did this child participate in Hampton Nippers last year?
Does this child have an anaphylaxis or asthma action plan?

If yes, please email a copy of all action plans to the Director of Junior Activities.

Any additional information that might help us better engage with this child, such as accessibility or communication needs? Feel free to reach out to the Director of Junior Activities directly, if needed.

Is this child interested in additional sport and skill enhancement? (Age 7+) Check the box and a team manager will reach out to you.

Regardless of athletic ability, this is a great opportunity to build confidence and friendships while trying out new skills.

Photo Release
Hampton Life Saving Club takes photos and videos at Nipper activities to showcase in club newsletters, the website, etc. Please confirm that this child may be included in these images. Otherwise, please contact our club secretary to opt out.
